Camp Release Statement and Information: I certify as the parent/guardian of my camper(s) that my camper is aware of the physical components involved in basketball camp, and release Peaster ISD and all associated coaches and professionals from any and all liability for personal injury arising out of the applicant's participation in camp, and grant permission in case of injury, accident, or illness, for my camper to be treated by a licensed physician. I agree to allow the participant(s) listed above to participate in the photo opportunities associated with camp and give Peaster ISD permission to use photos of my child in marketing promotions (print, electronic, etc.) All prints, slides, and digital images belong to Peaster ISD solely and completely. Online registration for camp confirms agreement to this statement.
