Parent Consent: Please enroll my child in the PHS Mini Cheer Camp. I agree that no one affiliated with the camp (director, instructor, or any school official) can be held responsible for any injury that may occur during this camp. I authorize the director to act for me in any emergency that may arise from participation requiring medical attention. Photography Consent: I agree to allow the participant listed above to participate in the photo opportunities available at the mini cheer camp and give Peaster ISD permission to use photos of my child in any marketing promotions (print, electronic, outdoor advertising, and publications). All prints, slides, and digital images belong to Peaster ISD solely and completely. Completed registration confirms agreement with all statements listed.
